Anesthetic challenges in minimally invasive cardiac surgery

Abstract

Cardiac surgery is a highly invasive and complex field of medicine. With increasing patient need and newer technology, minimally invasive cardiac surgery (MICS) has evolved to include surgical approaches utilizing less invasive techniques than those of traditional cardiac surgery. While the fundamental principles of open cardiac surgery and MICS are similar, MICS has many short-term and long-term advantages over the traditional approach such as reduced patient discomfort, shorter ICU and hospital stay, and lower costs. Contemporary MICS can treat a variety of cardiac conditions, including classic valvular lesions and, less commonly, isolated coronary and right atrial lesions. Because of the shift toward MICS, the anesthesia provider has a greater role in the management of anesthesia during cardiac procedures and faces new challenges. The provider must be familiar with the variety of surgical incisions and approaches and the perioperative care of MICS patients. This chapter presents a broad overview of perioperative anesthetic management for MICS.
